P1 registration survey form

Dear all,

Yesterday, the pupils received a form titled "Sibling Survey for Phase 1 Registration of Children for Admission to Primary One in 2019" (blue colour paper).

For those who do not have any sibling going to P1 next year, tick the first box. Parents are to write name, signature and date at the bottom of the form.

For those who have a sibling going P1 next year, tick the box accordingly and provide the necessary documents stated in the form.
